Overview

The main objective is to ensure genuine, correct claims are paid to the right claimants at the right time

Part Liquidation
(Partial Maturity)

Things you will need:
  1. Submit photocopy of policy document along with completed discharge voucher, statement of claim form including means of identification such as driver's license, national identity card or international passport.
  2. Current Utility Bill or Bank Statement of Account, or Tenancy Agreement, or water bill (to confirm residential address)
  3. Download and fill Know Your Policyholder (KYP) Form
  4. You will be notified when your Cheque is ready

Full Maturity

Things you will need:
  1. Submit original policy document along with completed discharge voucher, statement of claim form including means of identification such as driver's license, national identity card or international passport.
  2. Current Utility Bill or Bank Statement of Account, or Tenancy Agreement, or water bill (to confirm residential address)
  3. Download and fill Know Your Policyholder (KYP) Form
  4. You will be notified when your Cheque is ready

Termination
(Surrender)

Things you will need:
  1. Send us a letter of notification to surrender/termination indicating the intention to disengage.
  2. Submit original policy document along with completed discharge voucher, statement of claim form including means of identification such as driver's license, national identity card or international passport.
  3. Current Utility Bill or Bank Statement of Account, or Tenancy Agreement, or water bill (to confirm residential address)
  4. Download and fill Know Your Policyholder (KYP) Form
  5. You will be notified when your Cheque is ready

Loan

Things you will need:
  1. Submit an application for policy loan (Download Loan Form Here)
  2. Submit original policy document along with completed discharge voucher, statement of claim form including means of identification such as driver's license, national identity card or international passport.
  3. Current Utility Bill or Bank Statement of Account, or Tenancy Agreement, or water bill (to confirm residential address)
  4. Completed Loan Agreement form should be included in the drop down
  5. Download and fill Know Your Policyholder (KYP) Form
  6. You will be notified when your Cheque is ready

Death Claim

Things you will need:
  1. Send us a letter of notification of death of the life assured.
  2. Submit the following documents.
    • - Original policy documents
    • - Medical certificate of cause of death
    • - Burial certificate
    • - Death Certificate issued by National Population Commission
    • - Police report (if death occurred by road accident)
  3. Complete the following forms;
    • - Statement of Claim and Discharge Voucher forms to be completed by the claimant
    • - Physician's Statement form to be completed by the deceased personal medical attendant or Doctor that treated the deceased before death occurs
    • - Identification Statement form (not to be completed by any relative or anyone interested in the claim)
  4. Current Utility Bill or Bank Statement of Account, or Tenancy Agreement, or water bill (to confirm residential address)
  5. Download and fill Know Your Policyholder (KYP) Form
  6. You will be notified when your Cheque is ready
